    • Vision Insurance

      • Provider: Nippon Benefits · Network: EyeMed Insight · Eligible: Full-time employees

      • Annual eye exam: $10 copay in-network · Frames: $130 allowance (every 24 months)

      • Contacts or lenses: $130 allowance (annually) · Laser vision correction: 15% off retail

    • Basic Life and AD&D Insurance

      • Provider: Nippon Life Benefits · Benefit: Flat $50,000 — all eligible full-time employees

      • Accidental Death & Dismemberment (AD&D) benefit included at the same level

      • Accelerated death benefit included · Age reduction schedule applies at age 65+

  •  Company Match

    • UD UP TO 4% Match

      • 100% match on the first 3% of compensation contributed · 50% match on the next 2% = 4% maximum company match.

      • Employee contributions & safe harbor match contributions: 100% vested immediately.

  • Eligibility & Vesting

    • Must be 18+ years of age · Entry: first of month following age 18 and 90 days of service.

    • Pre-tax and/or Roth deferrals available (subject to IRS limits) · Rollovers from prior plans accepted.

  • Profit Sharing

    • Discretionary contribution at UD’s discretion · Separate from the 401(k)-company match.

    • Eligibility begins after 90-day probationary period · Team members must be in good standing and active at time of payout to receive.

    • Eligible employees may participate in our profit-sharing program. Based on recent company performance, a typical eligible first-year employee earned approximately $3,400 annually in profit-sharing payouts. Amounts vary and are not guaranteed.

These plans are 100% employee-paid — UD does not contribute to supplemental coverage. Benefits pay cash directly to you (not to a provider), and all plans are fully portable if your employment status changes.

  • Accident ( Assurity)

    • Cash benefits for covered accidents: ER visits, ambulance, X-rays, surgery, physical therapy, lodging, and more.

    • Initial treatment: $150–$300 · Ambulance: $300 ground / $900 air · Blood/plasma: $900 · Diagnostic exams: $15

  • Critical Illness (Assurity)

    • Lump-sum payout upon diagnosis: heart attack, stroke, cancer, kidney failure, organ transplant, Alzheimer’s, paralysis, coma, and more

    • Additional diagnosis and reoccurrence benefits included · Return of premium for non-critical-illness death

  • Hospital Indemnity (Assurity)

    • Hospital admission: $2,000 lump sum · Daily confinement: $200/day (up to 30 days) · ICU: $400/day (up to 10 days)

    • Inpatient surgery: up to $500 · Outpatient surgery: up to $200 · Anesthesia: $100

  • Short-Term Disability (Assurity)

    • Pays a weekly benefit when you are unable to work due to a covered off-the-job illness or injury

    • Partial return-to-work benefit: 50% of weekly benefit · Childbirth: 6 weeks (non-C-section) / 8 weeks (C-section

  • Cancer (Aflac)

    • Initial diagnosis: $5,000 (employee/spouse) / $10,000 (dependent child)

    • Radiation/chemo: up to $1,600/month · Annual care: $500/year up to 5 years · Surgery, hospitalization, and more covered
